The relative influence of the postal or genetic code “is incredibly nuanced. It depends on what disease you are interested in,” said Chirag Patel, a scientist at Harvard Medical School who oversaw the study led by Research Associate Chirag Lakhani.

The thoughtful design of the study included a massive review of twins from the insurance company database, but it also had a limitation – the researchers did not have information about which twins were identical, that is, they had 100% the same DNA, and which twins were fraternal, therefore, were not genetically identical.

The scientists had to determine which twins were identical using a statistical method.

Overall, their findings were in line with past studies of twins, which usually looked at the role of genes and environment in the development of individual diseases, but the new study found that, on average, diseases were slightly less influenced by genes and slightly more influenced by the environment.

An unsatisfactory answer to the question of which is more important: genes or environment, may simply be that both matter, and the contribution of each will vary depending on the disease.

“Innate through acquired; it’s a clear relationship,” said Tim Spector, a professor of genetic epidemiology at King’s College London who specializes in twin research and was not involved in the study.

Spector notes that the new study is the largest known review of American twins and has made valuable contributions to research on the subject with such a large sample size.

But he also pointed to another limitation: Because of the structure of American health insurance, only twins under the age of 24 could be studied. They had to be on their parents’ insurance for three years to be included in the database and identified as twins.

He noted that some diseases have a significant genetic component, but people with the same DNA die at different ages and from different diseases.

“I think people need to realize that you have a certain genetic soup that predisposes you to something, but life throws ingredients into it that are sometimes even invisible, but in the end you get a completely different end result,” Spector said.

Overall, the diseases studied were 31% hereditary, and 9% were determined by the environment of the twins.

Most of the differences remain unexplained. This may be due to the characteristics of individual twins, random factors, erroneous or incomplete data.

Melody Goodman, a biostatistician at NYU’s College of Global Public Health, notes that the study shows an excellent way to use the data set, but because it represented a commercially insured population that received mostly insurance coverage from their employers, it is unlikely to fully cover one of the largest categories of public health – the poor.

Goodman explains that genetic tools are highly valued as new research covers more and more diverse topics, but many of the factors that affect people’s health are already known.

“Even if we can fix all these genetic factors,” Goodman said, “it won’t eliminate the impact of poverty on people’s health.”
