Worldwide Cautions provide U.S. citizens with general information regarding terrorist activities, political violence, and criminal activity that occur abroad, as well as specific recommendations on how to prepare for possible contingencies, receive information on breaking security events and ensure that travelers can be contacted in an emergency. This version replaces the Worldwide Caution dated January 15, 2019.

The Department of State remains concerned about the continued threat of terrorist attacks, demonstrations, and other violent actions against U.S. citizens and interests overseas. The Department of State believes there is a higher potential for anti-American violence given the death of Ayman al-Zawahiri on July 31, 2022. Current information suggests that terrorist organizations continue to plan terrorist attacks against U.S. interests in multiple regions across the globe. These attacks may employ a wide variety of tactics including suicide operations, assassinations, kidnappings, hijackings, and bombings.

As terrorist attacks, political violence (including demonstrations), criminal activities, and other security incidents often take place without any warning, U.S. citizens are strongly encouraged to maintain a high level of vigilance and practice good situational awareness when traveling abroad.  U.S. government facilities worldwide actively monitor potential security threats and may temporarily close or periodically suspend public services to assess their security posture.  In those instances, U.S. embassies and consulates will make every effort to provide emergency services to U.S. citizens.  U.S. citizens abroad are urged to monitor the local news and maintain contact with the nearest U.S. Embassy or Consulate.  

When planning a trip and prior to departing the United States, U.S. citizens should consult country specific Travel Advisories and information pages on travel.state.gov.  Travelers are also urged to enroll in the Smart Traveler Enrollment Program (STEP) to receive security messages and to make it easier to locate them in an emergency.  The Department uses these security messages to convey information about terrorist threats, security incidents, planned demonstrations, natural disasters, etc.  In an emergency, please contact the nearest U.S. Embassy or consulate or call the following numbers:  1 (888) 407-4747 (toll-free in the United States and Canada) or 1 (202) 501-4444 from other countries.

Private U.S. citizens should not travel to any country to participate in armed conflict.  U.S. citizens are reminded that fighting on behalf of, or providing other forms of support to, designated terrorist organizations can constitute the provision of material support for terrorism, which is a serious crime that can result in penalties, including prison time and large fines.

Do Not Travel to Belarus due to the arbitrary enforcement of laws, the risk of detentionthe Russian military attack on neighboring Ukraine, and the buildup of Russian military in Belarus along the border with Ukraine. U.S. citizens in Belarus should depart immediately via commercial or private means.

On February 28, 2022, the Department of State ordered the departure of U.S. government employees and the suspension of operations of the U.S. Embassy in Minsk. All consular services, routine and emergency, are suspended until further notice. U.S. citizens in Belarus who require consular services should try to leave the country as soon and as safely as possible and to contact a U. S. embassy or consulate in another country.

U.S. citizens in Belarus who need assistance can also call the Department of State at 1(606) 260-4379 (from outside the United States) or e-mail [email protected].  For inquiries from within the United States, please call 1-833-741-2777.

Due to Russia’s unprovoked attack on Ukraine from Belarus, U.S. citizens located in or considering travel to Belarus should be aware that the situation is unpredictable and there is heightened tension in the region.   Potential harassment targeted specifically at foreigners is also possible.  Given the heightened volatility of the situation, U.S. citizens are strongly advised against traveling to Belarus.

The U.S. Embassy reminds U.S. citizens to exercise increased awareness and vigilance regarding political and military tensions in the region.  U.S. citizens are advised to avoid public demonstrations and to regularly reevaluate possible departure plans in the event of an emergency.

Country Summary: Belarusian authorities have detained tens of thousands of individuals, including U. S. citizens, for alleged affiliations with opposition parties and alleged participation in political demonstrations. U.S. citizens in the vicinity of the demonstrations have been arrested. Some have been victims of harassment and/or mistreatment by Belarusian officials. Belarusian authorities have targeted individuals associated with independent and foreign media. On May 23, 2021, Belarusian authorities forced the landing of a commercial aircraft transiting Belarusian airspace in order to arrest an opposition journalist who was a passenger.

Belarus enforces special restrictions on dual U.S.-Belarusian nationals and may refuse to acknowledge dual U.S.-Belarusian nationals’ U.S. citizenship, including denying or delaying U.S. consular assistance to detained dual nationals.

The Federal Aviation Administration (FAA) has issued an Advisory Notice to Air Missions (NOTAM) prohibiting all U.S. air carriers and commercial operators, U.S. airmen, and U.S. registered aircraft from operating at all altitudes in the Minsk Flight Information Region (FIR) (UMMV). What happened?

In the period from January 24 to 27, six patients were admitted to the intensive care unit of the City Consultative and Diagnostic Center No. 1, located on Siqueiros Street in St. Petersburg. All patients had previously undergone a gastrography procedure (examination of the stomach – note “VM” ). On the morning of January 28, it became known that three of them had died, and doctors were still fighting for the lives of the rest.

Shortly after the news of the death of patients, a message appeared on the website of the medical center that they had stopped accepting patients “for gastric examination using barium sulfate. ” Also, the staff of the institution asked all patients who had previously undergone a similar procedure and feel unwell to contact them.

In turn, the head doctor of the institution Evgeny Popov called the incident a tragedy. According to him, gastrography has been carried out in the diagnostic center since its opening and there were no complications in patients before.

— Research has been temporarily suspended at the moment. The investigation is under investigation. The drug that was used was sent for examination. The actions of doctors are also being checked, Popov said.

On the same day, January 28, the Investigative Committee opened a criminal case under two articles at once – “Causing death by negligence” and “Provision of services that do not meet the requirements of the safety of life and health of consumers.” At first, the head physician of the medical center was involved in the case as a witness, but soon his status changed to a suspect – Popov was detained.

Roszdravnadzor has also begun its investigation into the incident. The department announced the suspension of the circulation of the drug used in the examination of the stomach.

Versions of what happened

The Investigative Committee believes that for a long time the medical center headed by Popov used a “counterfeit chemical” instead of a certified one during examinations. Because of this, the health of more than 20 patients was harmed to varying degrees of severity, and three more clients of the institution died.

– In January 2022, six patients were admitted to the hospital, three of whom, despite the measures taken to provide medical assistance, died, the UK said in a statement.

Probably the counterfeit drug in question was barium sulfate. It is this contrast agent that is used in gastrography. Patients consume it in the form of a suspension before the start of the examination.

According to the 112 Telegram channel, two people died before the procedure began. The doctor may have confused barium sulfate with another drug, or the substance may have been diluted incorrectly.

The head of the “Young Chemist” laboratory Denis Zhilin noted that barium is indeed used for gastrography. But not simple barium, but its sulfate (salt of sulfuric acid – note “VM” ). It is barium sulfate that is a poorly soluble and safe substance for the human body. But other salts – nitrates and chlorides – are soluble and poisonous.

— In order to poison yourself to death, you need to drink barium chloride, that is, soluble salt, instead of barium sulfate. But in general, the lethal dose is quite high. For fatal poisoning, you need to eat at least about a gram, – Zhilin is quoted by 360.

The specialist also suggested that the poisonous substance could have got into the X-ray room through the fault of the powder manufacturer. In his opinion, the supplier could “slip barium chloride or other soluble salt instead of barium sulfate.”

Who supplied the medical center with barium?

Barium was supplied to the City Consultative and Diagnostic Center No. 1 by Neva Reaktiv LLC. The company emphasized that they are working with industrial barium, which is not intended for medical procedures.

The head of the company, Arkady Kamchatov, noted that the contract for the supply of barium with the medical center had indeed been concluded, moreover, within the framework of a state contract. However, it did not indicate that the barium supplied by the company would be used for medical purposes.

— We do not supply pharmaceutical substances. We supply chemical reagents, and we have this barium sulfate – not for fluoroscopy. This is a purely chemical reagent for industrial and analytical purposes,” Kamchatov said on REN TV.

He also added that representatives of the UK have already come to the company. However, Kamchatov himself sees the cause of the tragedy in the “unprofessionalism of the diagnostic center.”

The story of one patient

The wife of one of the deceased patients Alexandra said that her 42-year-old husband came for examination with gastritis on the morning of January 24th.
