Ciales puerto rico: Things to Do in Ciales

Things to Do in Ciales

From the Central Mountains to El Yunque National Forest, Puerto Rico has many waterfalls to explore.

Coffee farms, waterfalls, and caverns are part of the magic of Ciales.

Ciales is a picturesque and peaceful coffee town located in Puerto Rico’s Central Mountain Range and surrounded by thick vegetation. Here are a few of the things you should do when you visit.

Las Delicias Falls, in the beautiful Tres Picachos State Forest.

Waterfalls and Forests

If you like to be in touch with nature, take a walk through town to reach Las Delicias Falls, in the photogenic Tres Picachos State Forest, which looks like it was plucked from a tropical fairytale. The road to the waterfalls is long and has many curves but, once there, the effort will be justified.

When you arrive, park at the edge of the road and walk towards the first waterfall and its natural pool. Continue over some rocky terrain and you will reach Las Delicias.

There is also the famous Chorro de Doña Juana, which is on the border between Ciales and Orocovis. The waterfall can be seen from the road and is a wonderful backdrop photos. Many people will also enjoy a swim in the natural pool, although there are no changing areas or bathrooms, so be prepared in advance.

Exploring Caves

Ciales also has cave systems where you can book guided tours for rappelling, canyoneering and other adventures. The Yuyú Cave is known for its stalagmites, stalactites, corridors, and subterranean waterfalls.

Museo del Café is a museum and a coffee bar!

Museo del Café

This charming location belongs to a family that has grown coffee in Puerto Rico for more than 80 years. The museum is located near the town plaza, on the Paseo Aroma de Café, and in addition to being a museum, it is also a coffee bar that serves fresh brews, pastries, and features a shop with coffee-centric products.

The owner offers guided tours of the property where he explains the origin of the objects used in the coffee production process, and you can taste fresh Don Pello coffee grown in Ciales. Even if you want to buy coffee to go, they will roast beans just for you. During the weekends they offer barista courses and talks on topics related to the product. The Coffee Museum is open from Tuesday to Sunday from 8 a.m. to 3 p.m. and the entrance is free.

A gorgeous tree-lined path at Paseo Lineal Juan Antonia Corretjer in Ciales.

Film shell: Opadry yellow (Y-30-12863-A) (lactose monohydrate – 3.72 mg, hypromellose – 2.59 mg, titanium dioxide – 1.54 mg, triacetin – 0.74 mg, iron dye yellow oxide – 0.16 mg) – 8.75 m

Pharmacological effect

Improving erectile function

Pharmacokinetics

The pharmacokinetics of tadalafil in patients with erectile dysfunction is similar to the pharmacokinetics of the drug in individuals without erectile dysfunction. Absorption Tadalafil is rapidly absorbed after oral administration. Average Cmax in plasma is achieved on average 2 hours after ingestion. The rate and degree of absorption of tadalafil do not depend on food intake, so Cialis can be used regardless of food intake. The time of administration (morning or evening) did not have a clinically significant effect on the rate and extent of absorption. The pharmacokinetics of tadalafil in healthy individuals is linear with respect to time and dose. In the dose range from 2.5 to 20 mg AUC increases in proportion to the dose. The distribution of Css in plasma is achieved within 5 days when taking the drug 1 time / day. The average Vd is about 63 liters, which indicates that tadalafil is distributed in body tissues. Plasma protein binding of tadalafil at therapeutic concentrations is 94%. In healthy individuals, less than 0.0005% of the administered dose was found in semen. Metabolism Tadalafil is mainly metabolized with the participation of the CYP3A4 isoenzyme. The main circulating metabolite is methylcatecholglucuronide. This metabolite is at least 13,000 times less active against PDE5 than tadalafil. Therefore, the concentration of this metabolite is not clinically significant. Withdrawal In healthy individuals, the average clearance of tadalafil when taken orally is 2. 5 l / h, and the average T1 / 2 is 17.5 hours. Tadalafil is excreted mainly in the form of inactive metabolites, mainly with feces (about 61% of the dose) and, to a lesser extent, with urine (about 36% of the dose). Pharmacokinetics in special clinical situations Protein binding does not change with impaired renal function. Healthy elderly patients (65 years and over) had a lower oral clearance of tadalafil, which was expressed in a 25% increase in AUC compared with healthy individuals aged 19 and over.up to 45 years old. This difference is not clinically significant and does not require dose adjustment. In patients with mild to moderate renal insufficiency, dose adjustment is not required. Due to the increased exposure of tadalafil (AUC), patients with severe renal insufficiency are not recommended to use Cialis. The pharmacokinetics of tadalafil in patients with mild to moderate hepatic insufficiency is comparable to that in healthy individuals. No data are available for patients with severe hepatic impairment (Child-Pugh class C). When prescribing Cialis to patients with severe hepatic insufficiency, it is necessary to first assess the risks and benefits of using the drug. In diabetic patients on the background of the use of tadalafil, AUC was less by about 19% than in healthy individuals. This difference does not require dose adjustment.

Indications

Erectile dysfunction

Contraindications

Simultaneous administration of drugs containing any organic nitrates; children and adolescents up to 18 years of age; hypersensitivity to tadalafil

Use during pregnancy and lactation

The drug is not used in women

Dosage and administration

Int. The use of Cialis for erectile dysfunction (ED). For patients with frequent sexual activity (more than 2 times a week): the recommended frequency of administration is daily, 1 time per day, 5 mg, at the same time, regardless of food intake. The daily dose may be reduced to 2. 5 mg, depending on individual sensitivity. For patients with infrequent sexual activity (less than 2 times a week): it is recommended to prescribe Cialis at a dose of 20 mg, immediately before sexual activity, according to the instructions for the medical use of the drug. The maximum daily dose of Cialis is 20 mg. Use of Cialis for BPH or ED/BPH. The recommended dose of Cialis when used once a day is 5 mg; the drug should be taken at approximately the same time of day, regardless of the time of sexual activity. The duration of treatment is determined by the doctor individually. In patients with mild renal insufficiency (Cl creatinine from 51 to 80 ml / min) and moderate (Cl creatinine from 31 to 50 ml / min), dose adjustment is not required. In patients with severe renal insufficiency (Cl creatinine <30 ml / min and on hemodialysis): the use of Cialis is contraindicated

Side effects

Most common: headache, dyspepsia. Possible: back pain, myalgia, nasal congestion, flushing of the face. Rarely: eyelid edema, eye pain, conjunctival hyperemia, dizziness

Overdose

Symptoms: with a single administration of tadalafil to healthy individuals at a dose of up to 500 mg and to patients with erectile dysfunction – repeatedly up to 100 mg / day, undesirable effects were as follows same as when using the drug at lower doses. Treatment: in case of overdose, standard symptomatic treatment should be carried out. With hemodialysis, tadalafil is practically not excreted.

Interaction with other drugs

Tadalafil is mainly metabolized by the CYP3A4 enzyme. The selective CYP3A4 inhibitor ketoconazole increases AUC by 107%, while rifampicin reduces it by 88%. Although specific interactions have not been studied, it can be assumed that protease inhibitors such as ritonavir and saquinavir, as well as CYP3A4 inhibitors such as erythromycin and itraconazole, increase the activity of tadalafil. While taking an antacid (magnesium hydroxide / aluminum hydroxide) and tadalafil, the rate of absorption of the latter decreases without changing its AUC. Tadalafil enhances the hypotensive effect of nitrates. This occurs as a result of the additive action of nitrates and tadalafil on nitric oxide and cGMP metabolism. Therefore, the use of tadalafil in patients receiving nitrates is contraindicated

Special instructions

With extreme caution and only in case of emergency, it is possible to use tadalafil in patients with severe renal insufficiency (CC≤30 ml / min) and severe liver failure. During treatment with tadalafil, patients with moderate renal insufficiency (CC from 31 to 50 ml / min) were more likely to experience back pain compared with patients with mild renal insufficiency (CC from 51 to 80 ml / min) or healthy volunteers . In patients with CC≤50 ml/min, tadalafil should be used with caution. Therefore, the treatment of erectile dysfunction, incl. with the use of tadalafil should not be performed in men with heart conditions in which sexual activity is not recommended. The potential risk of developing complications during sexual activity in patients with diseases of the cardiovascular system should be taken into account: myocardial infarction during the last 90 days; unstable angina or angina that occurs during intercourse; chronic heart failure (functional class II and above according to the NYHA classification) that has developed within the last 6 months; uncontrolled heart rhythm disturbances; arterial hypotension (BP less than 90/50 mm Hg) or uncontrolled arterial hypertension; stroke within the last 6 months. Tadalafil should be used with caution in patients with a predisposition to priapism (for example, with sickle cell anemia, multiple myeloma or leukemia) or in patients with anatomical deformity of the penis (for example, with angulation, cavernous fibrosis or Peyronie’s disease). The patient should be informed of the need for immediate medical attention in the event of an erection lasting 4 hours or more. Untimely treatment of priapism leads to damage to the tissues of the penis, as a result of which a long-term loss of potency may occur. Against the background of the use of tadalafil, no cases of priapism were reported. It is not recommended to use tadalafil in combination with other treatments for erectile dysfunction

Clinical and pharmacological group:

Drug for the treatment of erectile dysfunction. PDE5 inhibitor

Pharmacotherapeutic group:

Erectile dysfunction treatment – PDE5 inhibitor

Pharmacological action

An agent for the treatment of erectile dysfunction, is a reversible selective inhibitor of specific PDE5 cGMP. When sexual arousal causes local release of nitric oxide, PDE5 inhibition by tadalafil results in an increase in cGMP levels in the corpus cavernosum of the penis. The consequence of this is the relaxation of the smooth muscles of the arteries and the flow of blood to the tissues of the penis, which causes an erection. Tadalafil is ineffective in the absence of sexual stimulation.

In vitro studies have shown that tadalafil is a selective PDE5 inhibitor. PDE5 is an enzyme found in the smooth muscles of the cavernous body, in the smooth muscles of the vessels of internal organs, in skeletal muscles, platelets, kidneys, lungs, and cerebellum.

The effect of tadalafil on PDE5 is more active than on other phosphodiesterases. Tadalafil is 10,000 times more active against PDE5 than against PDE1, PDE2, PDE4, which are localized in the heart, brain, blood vessels, liver and other organs. Tadalafil is 10,000 times more active at blocking PDE5 than PDE3, an enzyme found in the heart and blood vessels. This selectivity for PDE5 over PDE3 is important because PDE3 is an enzyme involved in cardiac muscle contraction. In addition, tadalafil is approximately 700 times more potent against PDE5 than against PDE6, which is found in the retina and is responsible for phototransmission. Tadalafil is also 10,000 times more active against PDE5 than its effect on PDE7-PDE10.

Acts within 36 hours. The effect appears already 16 minutes after ingestion in the presence of sexual arousal.

Tadalafil in healthy individuals does not cause a significant change in systolic and diastolic pressure, compared with placebo in the supine position (mean maximum decrease is 1.6/0.8 mm Hg, respectively) and standing (mean maximum decrease is 0.2/4.6 mm Hg. article respectively). Tadalafil does not cause a significant change in heart rate.

Tadalafil does not cause changes in color recognition (blue/green) due to its low affinity for PDE6. In addition, there is no effect of tadalafil on visual acuity, electroretinogram, intraocular pressure and pupil size.

There was no clinically significant effect on sperm characteristics in men who took tadalafil at daily doses for 6 months.

Pharmacokinetics

Tadalafil is rapidly absorbed after oral administration. C max is reached after an average of 2 hours. The rate and degree of absorption do not depend on food intake. The pharmacokinetics of tadalafil in healthy individuals is linear with respect to time and dose. In the dose range from 2.5 to 20 mg AUC increases in proportion to the dose. C ss in plasma are achieved within 5 days when taking the drug 1 time / day.

The pharmacokinetics of tadalafil in patients with erectile dysfunction is similar to the pharmacokinetics of the drug in individuals without erectile dysfunction.

V d is about 63 liters, which indicates that tadalafil is distributed in body tissues. At therapeutic concentrations, 94% of tadalafil binds to plasma proteins. In healthy individuals, less than 0.0005% of the administered dose was found in semen.

Metabolized with the participation of the CYP3A4 isoenzyme. The main circulating metabolite is methylcatecholglucuronide, which is 13,000 times less potent against PDE5 than tadalafil, so this metabolite is unlikely to be clinically relevant.

In healthy individuals, the average clearance of tadalafil when taken orally is 2.5 l / h, and the average T with urine (about 36%).

In patients with mild renal insufficiency (CC from 51 to 80 ml / min) or moderate severity (CC from 31 to 50 ml / min), AUC is greater than in healthy individuals.

Dosing regimen

The route of administration and dosing regimen of a particular drug depends on its form of release and other factors. The optimal dosage regimen is determined by the doctor. Compliance of the dosage form of a particular drug with indications for use and dosing regimen should be strictly observed.

Middle-aged men are recommended to take a dose of 20 mg at least 16 minutes before expected sexual activity, regardless of food intake. Patients may attempt intercourse at any time within 36 hours after ingestion in order to establish an optimal response time to tadalafil. The maximum frequency of admission is 1 time / day.

Side effects

Most common: headache, dyspepsia.

Possible: back pain, myalgia, nasal congestion, flushing of the face.

Rare: eyelid edema, eye pain, conjunctival hyperemia, dizziness.

Contraindications for use

Concomitant use of drugs containing any organic nitrates; children and adolescents up to 18 years of age; hypersensitivity to tadalafil.

Pregnancy and lactation

The drug is not used in women.

Use in hepatic dysfunction

With extreme caution and only in case of emergency, tadalafil may be used in patients with severe hepatic impairment.

Use for impaired renal function

With extreme caution and only in case of emergency, it is possible to use tadalafil in patients with severe renal insufficiency (CC ≤ 30 ml / min).

During treatment with tadalafil, patients with moderate renal insufficiency (CC from 31 to 50 ml / min) were more likely to experience back pain compared with patients with mild renal failure (CC from 51 to 80 ml / min) or healthy volunteers. In patients with CC ≤50 ml/min, tadalfil should be used with caution

Use in children

Contraindicated in children and adolescents under 18 years of age.

Special instructions

With extreme caution and only in case of emergency, it is possible to use tadalafil in patients with severe renal insufficiency (CC≤30 ml / min) and severe hepatic insufficiency.

The patient should be informed of the need for immediate medical attention in the event of an erection lasting 4 hours or more. Untimely treatment of priapism leads to damage to the tissues of the penis, as a result of which a long-term loss of potency may occur.

No cases of priapism have been reported during the use of tadalafil.

Tadalafil is not recommended in combination with other treatments for erectile dysfunction.

Drug interactions

Tadalafil is mainly metabolized by the CYP3A4 enzyme. The selective CYP3A4 inhibitor ketoconazole increases AUC by 107%, while rifampicin reduces it by 88%.
